Case Management
When you’re facing a serious injury, illness, or complex medical condition, it can be helpful to have a knowledgeable nurse to support you along the way. Our Case Management program connects you with a dedicated Registered Nurse. Your nurse works with you to help make the healthcare process clearer and easier to manage.
Your doctors remain responsible for your medical treatment and recommendations. Our role is to provide support, education, and coordination so your experience is less stressful and easier to manage.
Participation is free and completely voluntary when your plan offers this benefit.
Your nurse works with you to:

- Explain medical information and treatment options in clear, simple terms
- Help you navigate the healthcare system and your plan benefits
- Coordinate communication with your providers when appropriate

Chronic Condition Management
Living with a chronic condition such as diabetes, heart disease, or lung disease can be challenging. Our Chronic Condition Management program offers ongoing support to help you better understand your condition and take steps toward improved health.
As a participant, you’ll have access to a Registered Nurse.
Our goal is to support your overall well-being and help you feel confident managing your health over time.

Your Registered Nurse can provide:
- Education about your condition and treatment plan
- Personalized coaching to support healthy habits
- Tips and resources to help reduce complications
- Assistance coordinating care with your providers
- Information about referrals, services, or equipment your provider may recommend
- Educational materials tailored to your needs
Preauthorization & Utilization Management
Some services require preauthorization (also called prior authorization) under your health plan. This process involves reviewing certain treatments or procedures before they are provided to confirm they meet your plan’s coverage rules and medical guidelines.
ICM’s clinical team follows your health plan’s policies and uses nationally recognized medical guidelines when conducting reviews. We do not make treatment decisions, and your physician remains responsible for recommending and providing your care.
Your health plan determines which services require preauthorization. We work directly with providers to complete reviews within required timeframes and aim to make the process as straightforward and transparent as possible.
Preauthorization helps:
- Clarify coverage before care is received
- Reduce the risk of unexpected costs
- Ensure services meet your plan’s established medical criteria
We support you by providing:
- Clear and timely communication about authorization status
- Explanation of next steps if additional information is needed
- Information about appeal rights if a service is not approved
- Referrals to other supportive programs when appropriate
Healthy Mother Baby / Maternity Management
Pregnancy is an important and personal journey. Our Healthy Mother Baby program provides caring, reliable support throughout pregnancy and the early months after delivery.
You’ll work one-on-one with a Registered Nurse experienced in obstetrics. This program is designed to provide education and support so you feel informed and confident throughout your pregnancy experience.
Participation is free and completely voluntary when your plan offers this benefit.
