Disease Management - Living Well with a Chronic Condition

The Disease Management program helps members with chronic conditions by providing information and support to help them take an active role in their own health. Our nurses work on an individual basis with these members, their families, and their doctors to create a custom plan to best fit the member's needs. This plan can consist of components such as:

  • Educational materials
  • Contact with the treating physician
  • Specialist referrals
  • Medical equipment recommendation
  • Home health services

Through these services, our Disease Management program can help a member take control of their chronic condition, leading to a healthy, full life.


What to Do

Participants for the Disease Management program are identified through our other programs. One of our nurses will then make contact with members who have conditions such as:

  • Diabetes
  • Respiratory diseases (e.g. asthma and emphysema)
  • Non-metastatic cancer
  • Heart disease

While the participants are identified through our screening, if you feel that you or a member of your family would benefit from the Disease Management program, feel free to give us a call. Participation with our Disease Management program is entirely voluntary. Remember, not all of our clients choose to use all of our programs, so check with your Human Resources Department to find out if this program is offered as a part of your benefit plan, and if it isn't, let them know if you are interested.
