Disease Management - Living Well
with a Chronic Condition
The Disease Management program helps patients
with chronic conditions by providing information and support
to help them take an active role in their own health. Our nurses
work on an individual basis with these members, their families,
and their doctors to create a custom plan to best fit the member's
needs. This plan can consist of components such as:

help a member take control of their chronic condition, leading
to a healthy, full life.
What to Do
If you have a patient you believe would benefit from our Disease
Management program, call our office at 1-800-862-3338, and
ask to speak to the Disease Management department. One of our
friendly staff will then take the necessary information from
you, and determine if Disease Management would be appropriate
for your patient. At the time of your call, please have the
following information available to expedite the process:

If one of your patients is currently a participant of the
Disease Management program, ask to speak to the patient's assigned
Nurse Specialist. Our Nurse Specialists are familiar with their
patients' conditions, so they are often able to give immediate
authorization for services that may otherwise have to go through
review, and are also able to help you coordinate the patient's
care with other providers, if necessary.
