Disease Management - Living Well with a Chronic Condition

The Disease Management program helps patients with chronic conditions by providing information and support to help them take an active role in their own health. Our nurses work on an individual basis with these members, their families, and their doctors to create a custom plan to best fit the member's needs. This plan can consist of components such as:

  • Educational materials
  • Contact with the treating physician
  • Specialist referrals
  • Medical equipment recommendationHome health services

Participants for the Disease Management program are identified through our other programs. One of our nurses will then make contact with members who have conditions such as:

  • Diabetes
  • Respiratory diseases (e.g. asthma and emphysema)
  • Non-metastatic cancer
  • Heart disease

Through these services, our Disease Management program can help a member take control of their chronic condition, leading to a healthy, full life.


What to Do

If you have a patient you believe would benefit from our Disease Management program, call our office at 1-800-862-3338, and ask to speak to the Disease Management department. One of our friendly staff will then take the necessary information from you, and determine if Disease Management would be appropriate for your patient. At the time of your call, please have the following information available to expedite the process:

  • Patient's Full Name
  • Member ID Number
  • Subscriber's Full Name
  • Group Name / Name of the Employer or Union Local that supplies the insurance
  • Group Number
  • Patient Address and Phone Number
  • Patient Date of Birth
  • Diagnosis with ICD-9 codes
  • Copy of Insurance Card
  • Chart Notes / Clinical Information

If one of your patients is currently a participant of the Disease Management program, ask to speak to the patient's assigned Nurse Specialist. Our Nurse Specialists are familiar with their patients' conditions, so they are often able to give immediate authorization for services that may otherwise have to go through review, and are also able to help you coordinate the patient's care with other providers, if necessary.
